Clean claim rate
CMS describes a clean Medicare claim as one that does not require external investigation or development before payment processing. In operational dashboards, organizations often use a related clean-claim measure to monitor how many claims pass required edits without correction or additional information. Document the exact local definition.
Example internal formula
Clean claim rate = clean claims on initial submission / total claims submitted x 100
If 9,400 of 10,000 submitted claims meet the organization's clean-claim definition, the rate is 94%.
First-pass / first-pass resolution
First-pass measures look at how many claims move through the intended process without avoidable rework. Some teams measure first-pass acceptance at the clearinghouse or payer front end; others measure first-pass payment or resolution. Name the metric precisely so staff know which stage is being measured.
Denial rate
Denial rate can be measured by claim count, service-line count or dollars. Use one definition consistently and distinguish denials from front-end rejections.
| Metric | What it can reveal |
|---|---|
| Initial rejection rate | Front-end data, format, eligibility or transmission problems. |
| Clean claim rate | Overall claim quality before manual correction. |
| First-pass payment/resolution | How often claims reach the intended financial outcome without rework. |
| Denial rate | Adjudicated claims that require investigation, correction or appeal. |
Segment the problem
Break the metric down by payer, denial category, location, provider, specialty, procedure family, registration team or submission source. A 5% overall denial rate is less actionable than knowing that one payer and one authorization category account for most denied dollars.
Quick knowledge check
Why are rejection rate and denial rate different?
Rejections generally occur before normal adjudication, while denials occur after the payer processes the claim or service.
What is the first step before comparing clean-claim rates between organizations?
Confirm that both organizations use the same definition and denominator.
Why segment a denial rate by payer or category?
To identify the specific source of rework and choose a targeted corrective action.
