Module 11 · Specialty Billing Fundamentals
Lesson 4 of 5

Orthopedics, Physical Therapy & Surgery Billing

Orthopedic practices often connect office visits, imaging, injections, surgery and rehabilitation. Billing teams therefore need to understand where therapy rules, surgical global periods and procedure-specific payer edits can change what is separately reportable or payable.

Physical therapy billing controls

Therapy claims may require discipline-specific modifiers and accurate units based on the service definition and applicable payer methodology. For Medicare, outpatient physical therapy commonly uses the GP modifier, and CQ identifies certain services furnished in whole or in part by a physical therapist assistant when the policy applies.

Timed services

Do not convert treatment time into units by guesswork. Follow the applicable code definition and payer methodology, document actual service time where required, and verify the payer's unit rules.

Global surgery

Medicare Physician Fee Schedule indicators identify whether a procedure has a 0-day, 10-day or 90-day global period, or whether the global concept does not apply. Related routine postoperative work may be included in the surgical payment, while certain unrelated or separately reportable services may require specific documentation and modifiers.

Episode example

An orthopedic patient has an office evaluation, surgery and postoperative rehabilitation. The billing team tracks authorization, global-period dates, postoperative services, therapy plan requirements and payer-specific modifier rules so each claim reflects its role in the episode.

High-risk mistakes

  • Billing routine postoperative work separately without checking the global package.
  • Using therapy units that are not supported by documented time/service.
  • Missing required therapy or assistant modifiers.
  • Submitting duplicate procedure lines when laterality or modifier logic is incorrect.
  • Failing to connect authorization across surgery and rehabilitation.
Medicare example: CMS currently identifies 000, 010 and 090 global-surgery indicators and maintains specific outpatient therapy modifier rules. Always verify the current code-year and payer policy before applying them.

Quick knowledge check

Why does the global surgery period matter to billers?

Because related pre/postoperative services may be included in the procedure payment and may not be separately payable.

What does Medicare modifier GP generally identify?

Services delivered under an outpatient physical therapy plan of care when the policy applies.

Should therapy units be calculated from memory?

No. Use documented service details and the applicable current payer methodology.
