Track more than a denial count
- Payer and plan.
- Denial category and reason.
- Service, specialty, location or provider when useful.
- Dollar amount and volume.
- Whether the denial was preventable.
- Resolution action and outcome.
- Time to resolve and aging.
Map the cause upstream

A practice sees repeated eligibility denials from one payer. Instead of only correcting each claim, the team reviews registration timing and discovers that coverage is often checked weeks before the visit but not reverified near the date of service. The prevention action is to add a defined reverification step for that workflow and then track whether the denial category falls.
