Credit Balances, Overpayments and A/R Control

A negative or credit balance can signal an overpayment, duplicate posting, payment reversal issue, incorrect transfer or another account problem. Credit balances should be investigated rather than ignored or automatically refunded.

Why credit balances occur

  • Duplicate insurance or patient payment.
  • Payment posted to the wrong account or line.
  • Incorrect contractual adjustment.
  • Primary and secondary payer overlap.
  • Patient paid before insurance adjudication.
  • Payer recoupment or reversal not posted correctly.
  • True payer or patient overpayment.

Investigation sequence

Identify credit→Review ledger→Match payments/remits→Verify responsibility→Correct posting if needed→Refund/report under policy

Controls matter

Before refunding, confirm the payment source, amount, claim adjudication, prior refunds, reversals and any offsets or recoupments. Use the organization's approval and documentation workflow so the same credit is not refunded twice.

Compliance reminder: Payer overpayments can carry specific reporting and return obligations. Production workflows should follow current payer, contract, federal/state and organizational requirements rather than a generic training rule.

Use A/R controls to find problems early

  • Daily or periodic batch reconciliation.
  • Unapplied-cash and credit-balance work queues.
  • Old A/R reports segmented by payer and reason.
  • Duplicate-payment and refund checks.
  • Trend review for recurring posting errors.
