Eligibility and Benefits Verification

Eligibility verification helps a practice confirm that insurance information is active and understand relevant benefits before or around the time services are provided.

What eligibility verification answers

Eligibility verification confirms whether the member appears active for the relevant date and can provide benefit information such as plan type, cost sharing, network information, referral requirements, authorization requirements and service limitations when available.

A practical verification workflow

  1. Confirm patient demographics: name, date of birth and other identifiers used by the payer.
  2. Confirm insurance details: payer, member ID, group number and subscriber relationship.
  3. Check effective dates: confirm coverage is active for the expected date of service.
  4. Review plan/network information: determine whether the provider and location appear participating for the plan.
  5. Review relevant benefits: copay, deductible, coinsurance and benefit limitations when available.
  6. Check administrative requirements: referral or prior authorization when applicable.
  7. Document the verification: record the date, source, reference number or representative details when appropriate under the organization's workflow.
Eligibility is not a guarantee of payment. A claim can still be affected by coding, documentation, medical-necessity rules, authorization, coordination of benefits, timely filing, coverage exclusions and other payer requirements.

Common mistakes

  • Copying an old insurance card without checking current coverage.
  • Using the wrong member ID or subscriber relationship.
  • Checking general eligibility but not the benefit relevant to the planned service.
  • Failing to verify network or referral requirements.
  • Not documenting how and when verification was performed.

Does active eligibility guarantee the claim will be paid?

No. Eligibility is only one part of claim payment requirements.

Why document a verification reference number when available?

It can help show what information was obtained and support later research if a claim issue occurs.

When should insurance information be rechecked?

Organizations should follow their workflow, but coverage should not be assumed to remain unchanged simply because the patient was previously verified.
