Credentialing Workspace Terms of Service
Effective 2026-10-02 · commercial workspace terms for Ultra Credentialing.
1. Service scope
Ultra Credentialing is a workflow and record-management service for provider credentialing, payer enrollment, credential evidence, tasks, approvals, client reporting and related administrative operations. It is not an EHR, clinical decision system, payer portal, licensing authority or primary-source verification authority.
2. Customer responsibility
Customers are responsible for the accuracy of information entered into the service, user access they authorize, payer submissions they make, credentialing decisions they approve, and compliance with contracts, laws and professional requirements that apply to their organization.
3. Prohibited data
Do not use the workspace to store patient information, Social Security numbers, payer portal passwords, tax IDs, bank-account details, payment-card data or clinical records. Credential files should contain only information reasonably necessary for provider credentialing.
4. Human review
NPI assistance, AI suggestions and document extraction are assistive features. They do not verify a credential or replace human review. Authoritative credentialing decisions remain the responsibility of authorized staff.
5. Accounts and security
Customers must maintain accurate account details, protect authentication factors, assign least-privilege access, promptly revoke access that is no longer needed, and notify support of suspected unauthorized access.
6. Billing and subscriptions
Paid subscriptions are processed through Stripe. Plan limits, billing interval, renewal state, cancellation timing and applicable promotions are presented in the service and Stripe-hosted checkout or billing portal. Taxes or other charges may be determined by the payment provider where applicable.
7. Availability, backups and integrations
Customers should use the provided export and backup tools according to their own continuity policy. Third-party services and integrations may be unavailable independently of Ultra. No uptime or recovery-time commitment is created unless separately agreed in writing.
8. Compliance-support features
Audit, retention, preservation, security and reporting functions are tools that can support an organization's controls. Use of the software does not by itself establish HIPAA, SOC 2, HITRUST, GDPR or other certification/compliance status.
9. Acceptable use
Do not attempt unauthorized access, bypass plan or role controls, interfere with the service, upload malicious content, misuse public-source data, or use secure links for purposes unrelated to legitimate credentialing operations.
10. Data portability and termination
Workspace owners can use the available export/offboarding tools subject to access controls. Environment-specific secrets, authentication credentials and payment-provider secrets are intentionally excluded from portable packages.
