CPT Basics for Medical Billers

CPT is central to professional-service billing. Billers should understand what CPT represents and how to recognize claim problems without substituting for documented coding decisions.

What CPT represents

Current Procedural Terminology (CPT) is maintained by the American Medical Association and is widely used to report medical procedures and professional services. It includes services such as evaluation and management, diagnostic testing, surgery and many other professional procedures.

How CPT reaches the claim

  1. The provider documents the service.
  2. The appropriate coding workflow assigns the procedure/service code based on the record.
  3. Charges, modifiers and units are reviewed.
  4. The billing system places the coded service on the claim.
  5. Payer edits evaluate the submission.

Common biller checks

  • Is the procedure/service code valid for the date of service?
  • Are required modifiers present and appropriately supported?
  • Are units reasonable and supported?
  • Do provider, place-of-service and claim-type details align with the billed service?
  • Is a denial truly a coding issue, or is it eligibility, authorization, filing or another billing issue?

Evaluation and management

Evaluation and management services are a major CPT area used for many patient encounters. Code selection follows current CPT and payer rules and should be supported by documentation. A billing team should not select a higher service level merely because a claim pays more.
